I had to fail a student who turned in an answer to a PhD exam question I had given that was totally wrong. It's part of being advanced to "candidacy" status after which they can go ahead and propse a topic followed by the last step in which they have to defend their thesis. As part of phd qualifiers, you can ask them basically anything in a certain area to see whether the student has what it takes to carry out high level research towards a doctorate.
