WHAT CAN WE LEARN FROM THE HISTORY OF HEALTH REFORM IN THE UK?

Some of the world's leaders in health reform and quality improvement
are heading to Australia as part of a series of workshops organised
by the Australian Healthcare and Hospitals Association, in conjunction
with various partners.

In the first of a series profiling some of these visitors and their
contributions to health reform, the AHHA's Terrie Paul describes
the formidable achievements of the surgeon and UK health reformer,
Professor Lord Ara Darzi.

Prioritising quality of care in health reform

Terrie Paul writes:

A Baghdad-born Armenian who grew up in Ireland, Professor Lord Ara
Darzi is one of the world's leading surgeons. He is a charismatic
Renaissance man who operates at 6am, pioneering robotic non-invasive
technology, but still finding time to be a formidable committee man.

Darzi's family fled the genocide by the Ottoman Turks during World
War I. He attended a Jewish school and at 17 went to study medicine
and surgery at the Royal College of Surgeons in Ireland. He quickly
became fascinated with minimally invasive surgery at a time when it
was viewed with suspicion.

After moving to the UK as a consultant surgeon in 1991, he wrote
a seminal paper about his first laparoscopic colorectal cancer
resection. By 1994, he was an honorary consultant at Imperial College
and the Royal Marsden. Knighted for his services to surgery in 2002,
he then became the Paul Hamlyn Chair of Surgery at Imperial College
and Professor of Surgery at the Institute of Cancer Research.

When Darzi was summoned to the UK Prime Minister's office in June,
2007, he thought he was going to get a slap for his radical ideas
for reforming the National Health Service (NHS). Gordon Brown, to
his astonishment however, asked him to be a Health Minister.

In 2008, he launched High Quality Care for All - a report that
inaugurated a sea change in government policy. While Darzi presented
his next stage review as an extension of Labour policy, the truth
was quite different.

He instilled in government policy and thinking a patient-centred and
scientific approach to health reforms. Market-based decisions were
replaced with quality considerations: effectiveness, safety, and,
above all, the patient experience.

This was a watershed for the time. Bureaucratic managerialism had
been replaced with a new ideal - medical professionalism and clinical
engagement.

Touted as "the non-political politician", Darzi put a halt to the
formula that buried the NHS in perverse incentives. He invited medics
to contribute to the design of their own targets and engaged them
in reform.

By moving the focus to quality of care after Labour's decade-long
obsession with quantity (anyone care for a slice of activity based
funding?) the morale within the NHS improved, as did its public
standing.

For the first time, NHS organisations were charged with publishing
quality accounts. Institutions and individuals were rated by how well
they performed, and not just how many patients they processed.

By continuing his clinical practice, he forged respect with the
public and alliances with those who could help him traverse political
territory. He saved a colleague's life using a defibrillator on the
floor of the House of Lords, and helped another stricken by kidney
stones when No 10 sent him on a 3am house call.

He contributed to on-line forums, asking questions on parent sites
like "What makes it difficult for you to access health services? and
" If you want to access services outside normal working hours, how
do you decide where to go and what to do?

Professor Sir Bruce Keogh, cardiothoracic surgeon turned NHS Medical
Director, agrees that Darzi's contribution to the quality agenda has
been huge: "What he has done very effectively is change the mindset."

After publication of his review, Darzi saw his job as mostly done, but
stayed for a further year to steer the implementation of his reforms.

Never having intended a long political career, he resigned in July
2009 to concentrate on his passion for advancing the revolution in
robotics and image-guided surgery.

Professor Lord Darzi is one of the world's leading surgeons and is a recognised authority in innovation in surgery, particularly in minimally invasive and robot-assisted surgery. Away from the operating theatre, he is also a leading political voice in healthcare and medicine.
Born in Iraq to Armenian parents, Lord Darzi studied at the Royal College of Surgeons in Dublin before moving to the UK where he has lived and worked ever since. A brilliant surgeon, he joined Imperial College London and within four years was Chair of Surgery and Head of Department, the first of many senior appointments.
Research led by Professor Darzi is directed towards achieving best surgical practice through both innovation in surgery and enhancing the safety and quality of healthcare. His contribution within these research fields has been outstanding, publishing over 600 peer-reviewed research papers. His internationally renown team lead science and engineering research covering areas including Medical Image Computing, biomedical engineering, clinical safety, and robotics and remote surgery.
Appointed Parliamentary Under-Secretary of State at the Department of Health by Gordon Brown, Lord Darzi has also served as the UK Global Ambassador for Health and Life Sciences, and Chair of NHS Global as well as a UK Business Ambassador. He has also been appointed to the Privy Council.
In recognition of his outstanding achievements in research and development of surgical technologies, Professor Darzi was elected as an Honorary Fellow of the Royal Academy of Engineering, and a Fellow of the Academy of Medical Sciences. He also holds Fellowships from institutions all over the world in surgery, medical sciences and engineering.
Professor Darzi holds the Paul Hamlyn Chair of Surgery at Imperial College London where he is Head of the Division of Surgery. He is an Honorary Consultant Surgeon at Imperial College Hospital and holds the Chair of Surgery at the Institute of Cancer Research. He was recently appointed as Chairman for the Institute of Global Health Innovation at Imperial College.

LORD DARZI BECOMES FOREIGN ASSOCIATE OF THE INSTITUTE OF MEDICINE

Professor Lord Ara Darzi has been elected to the Institute of Medicine
(IOM), the health arm of the US National Academy of Sciences.Membership
of the IOM is one of the highest honours in the field of health and
medicine.Professor Darzi, the Director of the Institute of Global
Health Innovation at Imperial, is one of 70 new members and 10 foreign
associates elected for their "outstanding professional achievement and
commitment to service," the IOM announced today."It is a great honour
to be elected as a foreign associate of the Institute of Medicine,"
Professor Darzi said. "It is a rare privilege and reflects the hard
work of the team that I work with."Established in 1970, the Institute
of Medicine is one of the National Academies - along with the National
Academy of Sciences, the National Academy of Engineering and the
National Research Council - and serves as a resource for independent,
scientifically informed analysis and recommendations on health issues.

"It is an honour to welcome our highly distinguished colleagues to
the Institute of Medicine," said IOM President Harvey V. Fineberg.

"These individuals have inspired us through their achievements in
research, teaching, clinical work, and other contributions to the
medical field. Their knowledge and skills will deeply enrich the IOM."

Professor Darzi holds the Paul Hamlyn Chair of Surgery at Imperial

Professor Darzi was appointed a member of Her Majesty's Most Honourable
Privy Council since June 2009. In September 2013 he was appointed
to lead a new clinically-led London Health Commission, which will
conduct an evidence-based investigation into healthcare provision
and resources for Londoners.

Japanese government honours Lord Darzi

The Director of Imperial’s Institute of Global Health Innovation (IGHI) has received one of Japan’s most prestigious awards.

Professor the Lord (Ara) Darzi of Denham received the Order of the Rising Sun, Gold Rays with Neck Ribbon, in recognition of his contributions to global health and the development of medicine in Japan 

One of the nation’s oldest and highest national decorations, it is awarded by the Emperor of Japan to non-Japanese citizens who have made outstanding contributions to their field. 

Lord Darzi said: “It is a great privilege to receive this honour from Japan, a country I hugely admire.” 

Lord Darzi has served as Director of the IGHI since 2010 and has transformed the Institute with a focus on driving innovation in healthcare. His career has focussed on achieving best surgical practice through innovation in surgery and enhancing patient safety and the quality of healthcare. 

He is globally renowned for leading the development of minimally-invasive surgery, as well as pioneering the development of robotic techniques to facilitate medical operations. Through this field of work he has worked to enhance the scientific partnership between Japan and the United Kingdom with his involvement with a number of academic institutions and the Ministry of Health.   

Lord Darzi hosted the first Global Ministerial Summit on Patient Safety at Imperial in 2016, which placed patient safety high on the global agenda. In 2018, Lord Darzi opened the third summit in Tokyo, hosted by the Government of Japan, and ministers signed the Tokyo Declaration committing to “high level political momentum” towards the delivery of safer care everywhere. 

Lord Darzi has previously been recognised for his efforts in global health, having been awarded the Qatari Sash of Independence in 2014 by His Highness the Emir, Sheikh Tamim bin Hamad Al Thani , and the Order of Honour by the President of Armenia in 2017.

Lord Darzi is the Executive Chair of the World Innovation Health Summit, an initiative dedicated to capturing & disseminating the best evidence-based ideas and practices in global healthcare. 

In 2002 he was knighted for his services to medicine and surgery he and has been a member of the House of Lords since 2007. He served as Labour minister of health from 2007-9. Lord Darzi served as the UK’s Global Ambassador for Health and Life Sciences from 2009-13. In 2014 he was awarded the Order of Merit by the Queen. 

Recently, Lord Darzi helped negotiate the release of two Reuters journalists in Myanmar, who had been held since December 2017 after they reported on the ethnic cleansing of Rohingya Muslims.
